FABOH 1992-2008
Fond du Lac Area
Businesses on Health (FABOH) is a Preferred Provider Network with 76
member organizations. In existence since 1992, FABOH has grown to over
36,000 covered lives in the Fond du Lac area. A significant development
occurred in 1998 with FABOH's endorsement and participation in the new
Fond du Lac Community Health Plan.
COMMUNITY HEALTH PLAN
The
Community Health Plan is a three-tier plan design that enables
employers and employees to access greater savings in their health plan
benefits cost, in exchange for increasing the channeling percentage in
the health benefit plan design. A typical FABOH employer pays 90/10 for
PPO and 80/20 for Non-PPO. To access Tier 2, the employer will pay
90/10 PPO and 70/30 Non-PPO. The plan design incentive is intended to
increase provider business and decrease employer and employee costs.

FABOH presently has Medical and Chiropractic Provider Networks. The Community Health Plan Provider Directory lists all local Physicians and other providers who have been approved as participating providers to the Fond du Lac Community Health Plan and FABOH. These providers have accepted lower fee schedules in exchange for greater access to FABOH employees and their family members.
