• Become a new FABOH provider! Download our CHP Criteria and review the requirements for admission to our network. If you are interested in pursuing membership with our network, download the Provider Application and return to FABOH along with a letter of intent and all documents specified in the CHP Criteria document. Then our Provider Relations Committee will begin the process of reviewing your application. Return application materials to:

• If you are an existing FABOH provider, download a Provider Update Form to provide us with:
  • Staff changes.
  • Address/telephone changes.
  • Anything else that affects your FABOH contract and provider directory listing.
